Your SRN number should be clearly marked on the packaging that you send to us.

a) This form will request an SRN number. Complete the form to get your case filed. You will receive and SRN number either by email or fax. Make sure that the equipment is safely packed for transit with the SRN number clearly marked on the outside of the box. Do not send cables or remote controls unless the equipment will not work without them. The engineer will evaluate the faulty equipment and an official quotation will be sent to you. b) Could you please accept or reject this estimate within 60 days. If we have not received a response from you within this time period, the estimate will be deemed expired and therefore assumed to be a rejected estimate. A charge of £55.00 will be made for a declined estimate plus carriage, administration and VAT. This is deductable from the cost of any repair that you proceed. On some occasions where specialist equipment or parts are required, this fee may vary All work is covered by a three month guarantee. Should any parts fitted or work taken fail it will be replaced or re-worked free of charge for three months from the date of completion. This warranty applies to only the specific parts and work at the time of the repair and would be void if the equipment is misused or abused in any way c) If you do not hold an account with Active Audio Visual, please send a cheque (with your SRN Number written on the reverse) for the full amount INCLUDING VAT, made payable to:- Active Audio Visual, 34 Sutton Avenue, Tarleton, Preston, Lancashire, PR4 6BB. No repair will be carried out prior to receipt of payment. Alternatively, you may wish to pay by Credit/Debit Card by supplying us with your card number and expiry date (and Switch Issue Number if relevant)
